The admissions process begins when you submit an online form to help you understand the benefits covered with you insurance plan, by calling an admissions specialist at (970) 340-2228 (recommended) or by filling out the form below. We'll have a conversation to better understand use history where a recommendation can be made by our medical and clinical professionals on the appropriate residential or outpatient level of care.

COVID Rapid Testing will be provided to all clients admitting into Harmony effective December 16, 2020.
